Well Child Checkups · Salt Lake County
5400 South runs from Taylorsville straight to our Cottonwood office door.
We plot your child's measurements on their own curve, examine them head to toe, test vision and hearing, complete the developmental or mental health screening for their age, review immunizations, and answer the questions you brought with you.
We handle Granite District school and sports forms at the same visit. There is no need to book a separate physical.
Your child will see a pediatrician or advanced practice provider at our Cottonwood office, and the same one year after year wherever we can manage it.
That provider is the hub. Referrals to our dietitians, lactation consultants and behavioral health team, or to a specialist elsewhere, all start and finish with them.
We are aiming at what has no symptoms: low iron, hearing loss, blood pressure that is too high for a fourteen-year-old, a vision problem your child has quietly worked around, or a mood that has been sliding.
This is also the visit where you get to ask about the ordinary things — sleep, screens, eating, siblings, behavior — the questions that never justify their own appointment but shape a family's year.
Come in by age rather than by how your child seems: newborn and two weeks, then 1, 2, 4, 6, 9, 12, 15, 18, 24, and 30 months, then annually from three through twenty-one.
For teenagers, a spring appointment is easier to get and less rushed than an August one, and the sports form lasts just as long.
Ring the office or send a request online. If mornings are hard, the last appointment of the afternoon is usually easier to get than the first of the day and works better around a school pickup.
Bring the immunization record if you are new to us, the forms, and your list of questions.
Head east to 301 West 5400 South in Murray. From Taylorsville City Hall the drive takes about eight to eleven minutes, straight east on 5400 South — the same road the whole way, with no freeway and no clever route to remember.
Our Salt Lake office at 3838 South 700 East takes about eleven to fifteen minutes: east on 3500 South from around Valley Fair, then south. It is the better option if your day carries on toward Sugar House or the university.
For most of the city there is no real contest. Cottonwood is closer, the drive is simpler, and 5400 South is a road you already use. Salt Lake is worth it only if you live at the northern edge of Taylorsville near 3500 South, where the two are close to even.
You choose who your child sees. These are the pediatricians and advanced practice providers based at the offices below — tap a name for their background, training and the ages they see.
